TO QUALIFY

Health Extensions Dog Food and Health Extensions Vitamins must both be fed to your puppy for its entire lifespan along with 1 other supplement of your choosing. This includes Health Extensions Joint Mobility, Nutridrops, or Healthy Skin and Coat. You must make these purchases once every 90 days from the Petland location that you purchased your puppy in. COVERAGE

At the time of your pet’s passing, Petland will provide you with store credit equal to the original purchase price toward the purchase of another puppy. To qualify, a letter from an accredited veterinarian must be provided verifying the cause of passing. This program may only be used toward the purchase of a new puppy and does not cover any medications, surgeries, or other treatments. Petland is not responsible for any medical bills related to your pet’s passing.

To maintain eligibility, the following must be met:

• Mobility and Skin & Coat products are sold in quantities that typically last 1–3 months.
• A purchase of these products must be made at least once every 3 months to remain enrolled in the program.

If Health Extensions Dog Food does not meet your puppy’s dietary needs, Petland will assist you in selecting an alternative dog food brand that works best. As long as the new food is purchased from the same Petland location where you originally purchased your puppy, your puppy will continue to be covered under the Lifetime Puppy Program. Health Extensions supplements must still be fed.

Enrollment in this program is non-transferable between Petland locations or to Petland Corporate.

Program Coverage:

You will receive full store credit toward the purchase of another puppy in the event of:

• Passing from a major hereditary or congenital condition that prevents your dog from living a normal life, after one year of purchase.
• Passing from old age.
